Parents going on class trips are requested to fill out the following forms and turn them into the classroom teacher, who will pass them along to the main office.
PA Criminal Record Check
- Fill out the form online at https://epatch.state.pa.us. (pay by credit card) OR
- Print and complete the form and mail it to the address on the form. (pay by certified check or money order)
PA Child Abuse Clearance
Print and complete the form. Send it, with the PA Criminal Record check and a money order, to the address listed on the form.
PA Driver Record ( To be completed only if you will be driving students on a trip)
Complete the form online at www.dmv.state.pa.us. Click on “Request Your Driver History” and then click on “10-Year History.” (pay by credit card)
