Medical forms
	A variety of forms need to be submitted regularly to the school nurse. Not all forms need to be filled out yearly; contact the school office if there are any questions about when forms are due.   PA Dept of Health Student Physical  form (k-6)      --required for K (or entrance to school) and 5th grade   PMFS Pre-k Physical form      -- required for pre-k   PA Dept of Health Dental form      --required for K (or entrance to school) and 3rd grade   PMFS short-term medication form    (second page of the document; to be attached with any medication that comes to school) For your reference, please review the PMFS Medication Policy   PMFS Asthma Action Plan     -- to be updated yearly with student's doctor   FAAN Allergy Action Plan      -- to be updated yearly with student's doctor
